All forms are provided in PDF file format. This means you will need the free software Adobe Acrobat Reader (version 3.0 or higher) in order to read the forms. Each form can be opened and printed out on your local printer, or you may download the file to your computer for later use. Remember - you will need Adobe Acrobat to open and use the forms we provide, and this software is provided free of charge by the software manufacturer Adobe Acrobat.

Membership Application & Petition for Certification Forms

	Petition for Certification Form (186 KB) This document will provide you with the forms needed to Petition for Certification. The Petition for Certification has been updated as of 5/30/2009. The old petition will be accepted through June 2009.

Chapter Formation Forms

	Chapter Formation In the formation of a chapter, what the national headquarters will do, and what is expected of the members seeking to form a chapter, is outlined in this document.
	Chapter Bylaw Requirements The requirements for Chapter Bylaws are outlined in this form. Members seeking to form a chapter must read, understand, and agree.
	Uniform Chapter Bylaws The Association has adopted a uniform set of bylaws to govern chapter operations. A copy of these uniform bylaws may be obtain through this document.
	Sample Articles of Incorporation Chapters are required to incorporate as an educational non-profit corporation in their respective jurisdiction. A sample set of articles of incorporation is provided to assist in this function through this document.
	Petition for Charter The form national headquarters requires to obtain a Charter authorizing the formation and operation of an ACFS Chapter is this document.
